An Explanation of Forms For Your Move-In

MOVE- IN DAY INFORMATION
This form requests information regarding your move-in day. If there are any changes, please notify our office as soon as possible. We want to ensure that there will be no scheduling conflicts and that all the proper information has been received.

CARD ACCESS REQUEST
At 575 at Katy Freeway one card is issued for both parking and building access. The forms are used to maintain our building security system.

No charges are made for the first access cards issued to 575 at Katy Freeway tenants. Thereafter, a $10.00 non- refundable fee is charged for each 575 at Katy Freeway replacement access card.

DOOR SIGN ORDER
Please fill out the enclosed form with the name of your company as you wish it to be displayed outside your suite door. Door signs must be ordered at least three weeks prior to your move in order for installation to occur upon occupancy.

AFTER HOURS EMERGENCY CONTACT L IST
These lists will be used by security in the case of after-hours emergencies. Security will only allow the desired action to take place with the approval from an authorized individual.

FLOOR RESPONSE TEAM
This form designates individuals from your staff who will serve as floor wardens in the event of a building emergency.

PHYSICALLY IMPAIRED INDIVIDUALS
Please list those individuals who may need assistance in case of fire or other emergency.

EMERGENCY PROCEDURES ACKNOWLEDGEMENT
Complete this form to acknowledge receipt of the Emergency Procedures information found in this manual.

Pertinent Information For Your Move-In

MOVING INSURANCE

When moving into 575 at Katy Freeway your moving company will be required to provide a Certificate of Insurance with coverage for General Liability, Auto Liability, Excess Umbrella Liability, and Worker's Compensation.

Please note that I-10 EC Corridor #2, LP and its shareholders, members, parents, subsidiaries, managers, partners, officers, directors, affiliates, representatives, employees, successors, assigns and agents; American Realty Advisors LP and its shareholders, members, parents, subsidiaries, managers, partners, officers, directors, affiliates, representatives, employees, successors, assigns and agents ; and CBRE, Inc., are to be named as additional insureds and I-10 EC Corridor #2, LP, American Realty Advisors LP, and CBRE, Inc., as the Certificate holder. An Additional Insured Endorsement must be provided as part of but separate from the Certificate in order for it to be acceptable.

For your convenience, a sample Certificate of Insurance with Endorsement form can be found in the Forms section.

Once completed, the insurance information may be faxed to 281.497.0139.

MOVE- IN HOURS
We request that your move be scheduled after 6:00 p.m. Monday through Friday, or anytime on Saturday and Sunday.

FREIGHT ELEVATOR (freight elevator is 8’  deep x 5’ 10” wide x 12’ high. The diagonal measurement is 14’)
The building is equipped with one freight elevator. The elevator will be made available for use during your move- in. Please contact the Property Management Office in advance to schedule use of the freight elevator.

Tenants and contractors are able to schedule the use of freight elevators beginning at 6:00 p.m. on weekdays, 7:00 a.m. on Saturdays and Sundays.

Management must receive all cancellation notices 24 hours prior to the scheduled time. All reservations must also have a specific ending time. In the event of non-use without cancellation, the charge is a 3-hour minimum or the amount of time scheduled, whichever is greater at current rates.

Please call our office if you wish to schedule a freight elevator. We will inform you if the elevator is already scheduled for that time and ask if you would like to be notified in the event a cancellation is received.

HOUSTON BUSINESS USE & OCCUPANCY PERMIT:
Most businesses are not required to have a license in Houston; however, each tenant is required to have a Use and Occupancy permit posted within the suite.
